What is the FDA Boxed Warning for Ciprofloxacin?

The FDA-approved Boxed Warning for ciprofloxacin (Cipro) currently states that fluoroquinolones, including ciprofloxacin, have been associated with disabling and potentially irreversible serious adverse reactions that have occurred together, including:

• Tendinitis and tendon rupture
 • Peripheral neuropathy
 • Central nervous system effects

Ciprofloxacin should be discontinued immediately if a patient experiences any of these serious adverse reactions and patients should be switched to a non-fluoroquinolone antibacterial drug unless no alternative treatment options are available.

Fluoroquinolones may also exacerbate muscle weakness in patients with myasthenia gravis. Ciprofloxacin should be avoided in patients with a known history of myasthenia gravis.
Because fluoroquinolones have been associated with serious adverse reactions, ciprofloxacin should be reserved for use in patients who have no alternative treatment options for:

• Acute exacerbation of chronic bronchitis
 • Acute uncomplicated cystitis
 • Acute sinusitis
