Reserve Med Supply ("RMS," "us," "we," or "our") respects your privacy. We developed this Privacy Policy to explain how RMS collects, uses, shares, and protects Personal Information (defined below), and your choices about the collection and use of Personal Information.
This Privacy Policy applies to Personal Information collected or processed through www.reservemedsupply.com and any other RMS-operated website or social media page that links to this Privacy Policy (collectively, the "Site").
What is Personal Information?
"Personal Information" is information that identifies or relates to a particular individual. Examples of Personal Information include, among others, name, email address, phone number, mailing address, online identifiers, and - because our Services involve facilitation of medical consultations - health-related information you provide in connection with a medical intake questionnaire ("Health Information"). Health Information is treated with heightened care as described below.
Types of Personal Information We Collect and Use
We may collect and store Personal Information that you provide to us online (e.g., while on the Site or in responding via email to a feature provided on the Site). We collect Personal Information from our users via the Site during registration and purchasing. Generally, this information includes name, phone number, and email address for registration or opt-in purposes, and name, shipping address, email address, and credit card information when purchasing our products or services.
We also facilitate the collection of the following types of Health Information when you submit a medical intake questionnaire: medical history, current medications, known allergies, relevant health conditions, and other information you provide in order to facilitate a physician consultation. This Health Information is collected directly by our licensed physician network partner through a secure integration on the Site. RMS does not access or retain this Health Information. Where a prescription is issued, relevant fulfillment information (such as name and shipping address) is transmitted to our licensed pharmacy partner.
We collect the following types of information automatically through your use of the Site: Cookies, Device Identifiers, and Similar Technologies. When you use or access the Site, we and our service providers use cookies, device identifiers, and similar technologies such as pixels, web beacons, and local storage to collect information about how you use the Site. We process the information collected through such technologies, which may include or be combined with Personal Information, to help operate certain features of the Site, to enhance your experience through personalization, and to help us better understand the features of the Site that you and other users are most interested in.
Most browsers provide you with the ability to block, delete, or disable cookies, and your mobile device may allow you to disable transmission of unique identifiers and location data. Session Replay and Analytics
We reserve the right to use third-party session replay, analytics, and similar technologies to record mouse-moves, clicks, scrolling, and keystrokes typed into non-sensitive fields so we can fix bugs and improve site usability. Payment, password, health intake, and other sensitive fields will not be captured. Recordings are stored in pseudonymous form and are not sold or used for advertising. Where such technologies are in use and provide an opt-out mechanism, instructions to opt out will be made available through this Privacy Policy or by contacting us at the email address below.
Disclosure of Personal Information
We will not disclose Personal Information except as set forth in this Privacy Policy or with your consent. Mobile data will not be shared or otherwise disclosed to third parties or affiliates for their marketing or promotional purposes. This section describes to whom RMS discloses Personal Information and for what purposes:
Healthcare Partners: Health Information you provide through the medical intake questionnaire is collected directly by our licensed physician network partner through a secure integration on the Site. Where a prescription is issued, fulfillment information (such as name and shipping address) is transmitted to our licensed pharmacy partner. Both partners handle Health Information and PHI subject to applicable healthcare privacy laws, including HIPAA.
Trusted Service Providers: We engage third parties to perform tasks on our behalf and to assist us in operating the Site and providing our products and services. This may include affiliates, service providers, consultants, advisors, marketing and advertising partners, and joint marketing partners. For example, we may engage third parties to process payments, perform order-related services, and perform other activities on our behalf. This may require us to share Personal Information (including financial information) with third parties, who may store and use such Personal Information. We take commercially reasonable steps to help ensure our service providers provide at least the same level of protection for Personal Information as we do; however, your Personal Information collected by such third parties is subject to the third party's privacy policy. We have no control over, and are not responsible for, such third parties' collection, use, and disclosure of your Personal Information.
Corporate Transactional Partners: If RMS sells or otherwise transfers or merges part or the whole of our business or assets to another organization, any information collected through the Site, including Personal Information, may be among the items sold or transferred.
Legal, Compliance and Regulatory Matters: We may disclose Personal Information at the request of law enforcement or government agencies; in response to subpoenas, court orders, or other legal process; to establish, protect, or exercise our rights; to defend against a legal claim; to protect the rights, property, or safety of any other person; or as otherwise required by law.
Health Information and HIPAA
RMS is not a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). RMS does not directly collect, access, or store protected health information (PHI). Health Information you submit through the medical intake questionnaire on the Site is collected directly by our licensed physician network partner, MD Integrations, through a secure integration. Our licensed physician network partner and our licensed pharmacy partner are responsible for HIPAA compliance with respect to the PHI they receive and maintain.
To exercise rights with respect to your PHI - including access, correction, accounting of disclosures, restrictions on use or disclosure, or to file a complaint regarding the handling of your PHI - please contact our licensed physician network partner or our licensed pharmacy partner directly through the contact information provided to you during your consultation or with your prescription. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr.
How We Protect the Confidentiality of Personal Information
We protect the confidentiality and security of information RMS obtains in the course of business. We use commercially reasonable safeguards, such as industry-standard encryption technology, to help keep information collected through the Site secure.
Despite these efforts to store Personal Information in a secure operating environment, RMS cannot guarantee the security of Personal Information during its transmission or its storage on our systems. Further, while RMS attempts to ensure the integrity and security of Personal Information, RMS cannot guarantee that our security measures will prevent third parties from illegally obtaining access to Personal Information. We do not represent or warrant that Personal Information about you will be protected against loss, misuse, or alteration by third parties.
Accessing, Updating, or Deleting Personal Information
If you would like to access, update, or delete Personal Information about you, you may submit a request by emailing us at help@reservemedsupply.com. We will promptly review all such requests in accordance with applicable law.
Opting Out of Receiving Electronic Communications
We may send notifications, promotions, or other information via email or text message ("Communications"). You may choose to stop receiving Communications by following the instructions provided in a Communication (for example, by clicking "Unsubscribe" in an email or replying STOP to an SMS text message). If a Communication is necessary for us to provide you a requested product or service, we may not be able to provide you with an opt-out.
The Reserve Med Supply mobile message service (the "Service") is operated by RMS. Your use of the Service constitutes your agreement to these terms. We may modify or cancel the Service or any of its features without notice. By consenting to our SMS/text messaging service, you agree to receive recurring SMS/text messages from and on behalf of RMS through your wireless provider to the mobile number you provided, even if your mobile number is registered on any state or federal Do Not Call list. Text messages may be sent using an automatic telephone dialing system or other technology. You understand that you do not have to sign up for this program in order to make any purchases, and your consent is not a condition of any purchase. Your participation in this program is completely voluntary. We do not charge for the Service, but you are responsible for all charges and fees associated with text messaging imposed by your wireless provider. Message frequency varies. Message and data rates may apply. You may opt-out of the Service at any time by replying STOP.
Children's Privacy
The Site and Services are not directed to individuals under the age of 18. RMS does not knowingly collect Personal Information from minors. If you believe we have inadvertently collected information from a minor, please contact us immediately at help@reservemedsupply.com.
Your State Privacy Rights
Depending on your state of residence, you may have additional privacy rights under applicable state law (including the California Consumer Privacy Act (CCPA), the Virginia Consumer Data Protection Act, and comparable laws). Telehealth Consent and Informed Consent Agreement
Effective: June 1, 2026
By submitting the medical intake questionnaire and proceeding with a consultation request, you confirm that you have read, understood, and agreed to this Telehealth Consent and Informed Consent Agreement. This Agreement is legally binding. Please read it carefully.
Nature of Telehealth Services
Telehealth involves the delivery of healthcare services, including medical assessment and the issuance of prescriptions, through electronic communications rather than an in-person visit. The physician assigned to your consultation is an independent contractor of our licensed physician network partner and is not an employee of Reserve Med Supply.
You understand and agree that:

You will not be in the same physical location as the reviewing physician.

The physician is licensed in the state(s) in which they are authorized to practice. You must accurately report your state of residence.

Telehealth has inherent limitations compared to in-person care, including the inability to conduct a physical examination, order diagnostic tests, or directly observe your condition.

The physician's assessment is based solely on the information you provide in the intake questionnaire. You are responsible for the completeness and accuracy of that information.

Telehealth services provided through this Site do not constitute an ongoing physician-patient relationship.

The physician may decline to prescribe for any reason consistent with their professional judgment and applicable law.
Risks of Telehealth

Limited diagnostic capability: the physician cannot conduct a physical examination, which may limit the accuracy of assessment.

Technology failures: technical issues, including connectivity problems, may interrupt or degrade the consultation process.

Privacy risks: while we use industry-standard security measures, electronic transmission of health data carries inherent residual risk.

Incomplete information: if you provide incomplete or inaccurate medical history, the physician's assessment may be based on incorrect assumptions, which could result in an inappropriate prescription.
Emergency Use Commitment
You expressly agree that you will use any medications prescribed through this Service ONLY in an emergency situation, meaning a situation in which you have made a reasonable, good-faith effort to obtain care from a licensed healthcare provider and such care is not reasonably available to you at the time.
You will NOT use the prescribed medications as a substitute for routine or non-emergency medical care. You will NOT use medications beyond their labeled or prescribed indications without first consulting a qualified healthcare professional.
IN CASE OF ANY MEDICAL EMERGENCY, CALL 911 OR GO TO YOUR NEAREST EMERGENCY ROOM IMMEDIATELY. THE MEDICATIONS IN YOUR KIT ARE NOT A REPLACEMENT FOR EMERGENCY MEDICAL SERVICES.
Medication Storage and Use

Store all medications in accordance with the storage instructions on the label and in the Reserve Med Guidebook (cool, dry place; protect from moisture; away from direct light).

Keep all medications out of reach of children and unauthorized persons.

Do not transfer, sell, or distribute any prescription medication to any other individual.

Consult the Reserve Med Guidebook and your prescribing physician's instructions before using any medication, and seek professional medical advice when available.

Contact a healthcare professional promptly if you experience any adverse reaction.
Medication Expiration and Shelf Life

Expiration dates indicate the date through which the manufacturer guarantees full potency under proper storage conditions. They do not necessarily indicate the date on which a medication becomes harmful.

You should check expiration dates periodically and arrange for replenishment before medications expire.

We recommend using medications within their labeled expiration dates.
Controlled Substances
No controlled substances (as defined under the federal Controlled Substances Act or applicable state law) will be prescribed through this Service. If you require a controlled substance, you must consult a healthcare provider in person.
Withdrawal of Consent
You may withdraw this consent at any time by contacting us at help@reservemedsupply.com. Withdrawal is effective prospectively and does not affect services already rendered or prescriptions already issued.
Liability Waiver
To the fullest extent permitted by applicable law, you release and hold harmless Reserve Med Supply, our licensed physician network partner, our licensed pharmacy partner, and their respective officers, directors, employees, agents, and affiliates from any and all liability, claims, damages, losses, or expenses (including attorneys' fees) arising from or related to: (a) the telehealth consultation; (b) the issuance or non-issuance of a prescription; (c) your use, misuse, or storage of the medications; or (d) any adverse reaction or harm resulting from use of the medications, except to the extent caused by gross negligence or willful misconduct.
